For more than 20 years, HomeTeam has provided home-based primary care for older adults across the Greater Victoria area. Our interdisciplinary team includes physicians, nurses, physiotherapists, and a rehabilitation assistant who work closely together to support patients in their own homes.

About the Role

This is a community-based nursing position focused on seniors living in their own homes or retirement residences. Patients vary in their level of independence and medical complexity.

This role does not involve long-term care work. Our patients remain in the community, and care is provided where they live.

The structure of the practice allows nurses to focus on quality care rather than volume. The goal is to improve quality of life, support caregivers, and help patients remain safely at home whenever possible.

Working closely with physicians and the interdisciplinary team, the LPN provides hands-on clinical care, monitoring, patient education, and ongoing support for older adults in the community.

Depending on experience and practice needs, the LPN may carry an independent caseload or provide standardized nursing assessments and follow-up care for existing patients.

This role is well suited to an experienced LPN who is comfortable working independently, exercises strong clinical judgment, and enjoys relationship-based care.


What You Will Do

  • Conduct patient assessments and collect health histories

  • Monitor and report changes in patient condition

  • Support management of acute and chronic health concerns

  • Perform clinical procedures such as wound care, injections, suture removal, and ear syringing

  • Administer vaccinations and medications within LPN scope of practice

  • Monitor nutrition, medications, and overall health status

  • Perform screening and preventive care activities

  • Collect specimens and complete point-of-care testing

  • Provide education and support to patients and families

  • Collaborate closely with physicians and the interdisciplinary team

  • Coordinate care with community and hospital partners

  • Support patients in maintaining independence and safety at home

Location and Logistics

  • Service area includes Victoria, Oak Bay, Gordon Head, Royal Oak, and Esquimalt

  • Mileage is reimbursed

Compensation and Benefits

  • Competitive salary based on experience

Requirements

  • Licensed Practical Nurse (LPN) licensed in British Columbia

  • Currently located in the Greater Victoria area

  • Minimum 3 years of recent clinical nursing experience preferred

  • Community care, primary care, or geriatrics experience is an asset

  • Comfortable working independently in a community setting

  • Strong communication and teamwork skills

  • Patient-centered approach to care

  • Valid driver’s license and access to reliable transportation
