About the Role

This is a community-based nursing position focused on caring for older adults living in their own homes or retirement residences throughout Greater Victoria.

Unlike many community nursing positions, this role is not focused on short-term visits or high patient volumes. Our model emphasizes continuity of care, allowing nurses to develop long-term relationships with patients and become a trusted part of their healthcare team.

You'll work closely with physicians, registered nurses, physiotherapists, and rehabilitation staff to support patients with a wide range of health needs, from preventive care and chronic disease management to acute concerns that arise in the community.

Most importantly, you'll have the time and support to deliver the kind of care that brought many nurses into the profession in the first place.

What You'll Do

  • Conduct patient histories and nursing assessments within LPN scope of practice

  • Monitor and support patients with acute and chronic health conditions

  • Perform clinical procedures including wound care, injections, suture removal, and ear syringing

  • Administer vaccinations and prescribed medications

  • Monitor nutrition, medications, and overall health status

  • Perform screening assessments and preventive care interventions

  • Collect specimens and complete point-of-care testing

  • Provide education and guidance to patients and family caregivers

  • Collaborate closely with physicians and other members of the interdisciplinary team

  • Coordinate care with community resources, specialists, and hospital partners

  • Advocate for patients and help support aging safely at home

Qualifications

  • Licensed Practical Nurse (LPN) licensed and in good standing with BCCNM

  • Currently residing in the Greater Victoria area

  • Valid driver's license and reliable vehicle

  • Clinical nursing experience required

  • Experience in geriatrics, primary care, community nursing, or long-term care, is an asset

  • Strong communication and interpersonal skills

  • Ability to work independently while collaborating effectively within a team

  • Commitment to patient-centered, relationship-based care
